Instructor Membership Agreement:
Applicant certifies that the information contained in this application is correct to the best of Applicant’s knowledge. Applicant understands that falsifying information is grounds for rejecting the application or termination of the service agreement between Mentor Society and Applicant.
Applicant must meet requirements to interact with students and before accepting funds through charter schools, including obtaining a LiveScan clearance. Applicant authorizes any person, organization, or company listed on this application to furnish you with all information concerning Applicant’s previous employment, education, and qualifications. I also authorize you to inquire about and receive such information.
Nothing on this application is intended to create or imply a contractual relationship. If accepted, the Applicant understands that Applicant’s service with Mentor Society may be terminated, or any offer or acceptance withdrawn, at any time, by either Applicant or Mentor Society, for any reason not expressly prohibited by law.
All curriculum and content must be reviewed for appropriateness before being approved for use on the Mentor Society LMS platform.
In consideration of this agreement, Applicant agrees to abide by the rules and regulations of Mentor Society, which may be changed, withdrawn, added, or interpreted at any time, at Mentor Society’s sole option and without prior notice to Applicant.
